Below is a copy of the form that we use to fill out when a student/patron is injured in the library.  There are copies in the bottom right hand drawer of Jared's Desk.  Be sure and call the PCC Police (x7484) IMMEDIATELY and have them come to assist in the filling out of the form.  Also, call/email Jennifer Cooper with a description of what happened.
